Sports Injury Procedure (to be completed by injured person)
- Goto football.claimsgateway.com
- Create/Sign-up for account if claiming for first time
- Complete the online claim form (providing details of injury and activity)
- Add additional documentation requested
- Generate Club report and submit to Club Secretary to complete and sign
- Upload club report.
